Request Medical Equipment
Please call first to see if we have your requested item/items in stock. If so, you may pick up your item/items at our warehouse during open hours. Each recipient is asked to fill out a short request/general information form and a signature is required for a Liability Waiver (see below). Basic training is given for the specific piece of equipment for increased safety. Further equipment recommendations may also be given based on each individual’s needs. Items are gifted to the recipient at no cost from All Blessings Flow. If the recipient is able to make a donation to our mission to help towards operation expenses we would greatly appreciate the donation. If we do not have the item requested, we will make every effort to find the equipment that is needed through our network of partners.
If at all possible, please print this form and complete as much of it as you can.

Donate Medical Equipment
If you have gently used medical equipment you wish to donate you may drop off your donations at our warehouse at Riverside Center during open hours. Drive down behind UVA Primary Care and we are on the lower level on the backside. We are doing curbside service, so just honk your horn and someone will be right out to assist you. Please DO NOT leave items outside our door after hours.
|
We ask donors to be sure that the equipment is cleaned and in good working condition. We do not accept items that are no longer in working order such as an electric hospital bed or power chair/scooter. We do accept power wheelchairs/scooters that need working batteries, as long as it was operational when the batteries were still working.
